
We help older adults age 60 years or older access long-term services and supports to remain independent in their place of choice. Many of those aging in place face significant challenges including chronic health conditions, disability, poor nutrition, inadequate or costly housing, frailty, and behavioral health conditions. We offer a wide variety of programs and services designed to support the physical, behavioral, and emotional health of the older adults we serve. Our case managers work with older adults, their caregivers and family supports to assist with eligibility requirements and choosing appropriate services and supports based on each person’s individual needs and preferences.
