Are you caring for a child with special needs, or an older adult with a chronic health condition or complex care needs? For MassHealth members aged 3 to 64, our Long-Term Services and Supports (LTSS) Community Partner Program can help connect you and your loved one to long-term care services.

An individual may be qualified for the Community Partners Program if they experience:
- Brain injury or cognitive issues
- Physical, intellectual, and developmental disabilities (including autism)
- Long-term home and community-based needs
How Does It Work?
LTSS Community Partners will provide long-term support and services (LTSS) care coordination and navigation to certain members with complex LTSS needs, such as children and adults with physical disabilities, developmental disabilities, and brain injuries.
Care coordinators will work with you, your family, your care team, and your community to find the services that best fit your loved one’s individual needs. Your care coordinator will facilitate information and communication across all parties, ensuring that all decisions are made with the member’s wishes and care in mind.

Available Services
Some of the services we can help connect you to include, but are not limited to:
- Adult foster care
- Certified trainers in advanced health and wellness and disease prevention programs
- Family caregiver support and respite services
- Housing resources
- Personal care attendants
- Special programs offering care management
- Supplemental Nutrition Assistance Program (SNAP) enrollment
- Transitions of care services
- Durable medical equipment
- Department of Developmental Services (DDS), DSS, etc.
- Other needed resources
Working together, we offer a convenient way for you to access the community-based resources you need to remain independent and in the community.
Eligibility:
- MassHealth member enrolled in either an:
- Accountable Care Organization (ACO)
- Managed Care Organization (MCO)
- Ages 3-64
- Has complex medical needs, developmental or acquired disabilities, or a high need for long-term services and supports
